FHIAP Post-card Information
Have you received a post-card?
If you receive a post-card, it means we now have a place for you in FHIAP.
 
Call us at 1-888-564-9669, so we can:
  • Find out if you still want a FHIAP subsidy.
  • Make sure your contact information is correct.
  • Find out if you can still get health insurance at work.
You'll get an application when available. See checklist below to make the most of your time while you wait.

What's The Post-Card About?
FHIAP's waiting list is first-come, first-serve. Those with post-cards have a place available in our program. If you did NOT receive a post-card, it means it's not your turn, yet. Don't gather any information until you hear from us. We will notify you when your number is up. If you send us any information before we need it, we can't use it!
 
If you had a change in your contact information, let us know. Please call 1-888-564-9669, or e-mail us with your reservation number, and new information.

Checklist
Get ready for your application - gather the following:
  • A copy of your Oregon Driver License or other proof that you live in Oregon. Other proof can be a utility bill with your name and address or a rental agreement with your name and address.  
     
  • Proof of U.S. citizenship and identity (see flyer in English.pdf or Spanish.pdf). If you send your ODL as proof that you live in Oregon, it can also prove identity
     
  • A copy of both sides of your U.S. citizenship and Immigration Services card (used to be called your INS card) for each family member who is NOT a U.S. citizen.
     
  • Your employer’s ID number (also called an EIN number). You can locate this number on your W-2 form (copy your W-2 if you can’t find it) or ask your employer. 
     
  • Proof of income (such as pay stubs) for three months, for you and your spouse, if applicable. Make sure your pay stubs cover the three months before the month you sign.
     
  • A doctor’s note with due date if you are pregnant.
     
  • Proof of any deductions, such as child support payments you made.
     
  • Have your employer complete the Group Insurance Information form (.pdf).

For Self-Employed Applicants:
 
Please get the following for self-employed applicants:
  • Most recent federal tax return and all schedules if you or your spouse are self-employed, or if you are a farmer, fisherman, or rancher.
     
  • The Self-employment Income Worksheet and copies of your business ledgers and bank statements for six months if you or your spouse are self-employed.
     
  • The Farming, Ranching, and Fishing Income Worksheet and copies of your business ledgers and bank statements for 12 months if you or your spouse are self-employed as a farmer, fisherman, or rancher.
